Registration and Licence Application Forms
Regulation is achieved primarily by
- registration of your x-ray equipment, radioactive substances or electronic
products and the premises where they are manufactured, operated, used or stored; and by
- licensing individuals to possess, use, operate, manufacture or otherwise deal
with particular radiation sources.

Notes:
- The shortest period for which a licence or registration can be granted is 12 months. Where registration
or licence is required for a period of up to 3 months, an application may be made for a Temporary Permit.
- Renewal application forms are sent to the nominated mailing address one month prior to expiry. If you have
changed your address or have not received your renewal application forms upon expiry, please contact us.
